My Stuff – leaving care kit

Last Updated Sep 2011

The kit has been developed for young people as a planning tool and resource folder to help them prepare for transition through care. The topics in the kit are designed to be explored by the young person with appropriate support from their carer, social worker, youth worker, extended family members or a range of people from government and non-government agencies. As the young person investigates the topics they will build their knowledge about resources, contact people, benefits, etc related to the topic areas.
