Sector surveys will be conducted at two stages during the evaluation namely
- Stage 1 - November 2011
- Stage 2 - November 2012
At each stage there will be two separate on-line surveys;
Ø The service survey can be completed by Team leaders / Coordinators
Ø The worker/staff survey can be completed by staff involved in directly providing/delivering services to clients.
Stage 1 surveys can be completed during a three week period
Monday 14 November to Friday 2 December 2011.
A link to the on-line surveys will be emailed to services by Thursday 10 November 2011.
The on-line surveys will have the capacity to be saved and completed over time. The first time the survey is saved you will be provided with a unique code. This code will then need to be entered each time you return to your survey. Therefore it will be important to remember or record your code.
Please note: the code will not be connected to your submitted survey.
